Evidence-based practice models are procedural frameworks for moving research into clinical use. Most guides profile them one by one. That is not the useful question for anyone actually building the evidence base, because the choice of model quietly sets three things: how wide the search has to be, which appraisal instrument you are committed to, and what counts as sufficient evidence to act. This piece works from those consequences backwards.

Consequence one: how wide the search has to be

Models differ sharply in how much evidence they expect before a change is justified, and that expectation determines search breadth.

Frameworks organised around an organisational decision, the Iowa Model being the clearest case, ask whether the research base is sufficient at an explicit decision point. Sufficiency is judged against a body of literature, so the search has to be broad enough to support a claim about the state of the evidence, not merely to find supportive studies. In practice that means a structured search with documented yield, close to what a full systematic search requires.

Practitioner-oriented frameworks such as the Stetler model place the individual clinician at the centre and permit a narrower, more targeted search, because the decision being supported is smaller in scope. That is a legitimate difference, not a lower standard, but it must be stated rather than assumed.

PARIHS shifts the burden again. Because it treats context and facilitation as constitutive of successful implementation rather than background, it expects evidence about the setting alongside evidence about the intervention. That usually means grey literature and local data, and searching beyond the indexed databases becomes part of the method rather than an afterthought.

Consequence two: the appraisal instrument you are committed to

This is where the choice binds hardest, and where retrofitting is most expensive.

The Johns Hopkins framework carries its own appraisal apparatus, rating evidence for level and for quality separately. Adopting the model means adopting that rating scheme. You cannot sensibly declare the framework and then appraise with CASP checklists instead, because the model’s translation stage consumes ratings the checklists do not produce.

Models without a bundled instrument, including Iowa, Stetler, and ACE Star, leave the choice open, which means you have to justify it. For mixed designs the JBI suite covers more study types than most alternatives; for qualitative work CASP remains dominant. The distinction that catches people out is between appraisal and risk of bias assessment. Practice models generally ask for the former. If your work is heading toward publication as a review rather than a local project, reviewers will expect the latter, and the two are not interchangeable.

Consequence three: what counts as enough to act

Every model contains an implicit answer to the question of when evidence is strong enough to change practice, and the answers differ.

Decision-point models make the judgement explicit and collective: a team decides sufficiency at a named stage. Practitioner models locate the judgement with the individual, which makes it faster and less defensible. ACE Star treats the conversion of evidence into a usable guideline as the pivotal step, so the threshold question becomes whether the evidence is specific enough to yield a protocol.

None of these thresholds is a certainty rating in the formal sense. If your project needs a defensible statement about how confident the evidence permits you to be, that is GRADE certainty assessment, which sits alongside a practice model rather than inside it.

Choosing before you search, not after

The practical rule is that the model has to be settled before the search is run, because it determines search breadth, the appraisal instrument, and the sufficiency threshold. Selecting a framework after the evidence is assembled means either redoing the appraisal in a scheme the model expects, or declaring a model the work does not actually follow. Both are visible to a careful reader.
