Appraisal and synthesis
This is the clearest divide. A systematic review formally rates each study’s risk of bias and may pool results in a meta-analysis. A scoping review typically does not assess study quality and does not pool results; instead it presents a descriptive map, often as tables and charts showing how the evidence is spread. The output answers “what is out there” rather than “what is the answer.”
Reporting standards and registration
Both review types are reported against the relevant PRISMA guideline, with a scoping-review extension that reflects its mapping aim. Systematic reviews of health questions are typically registered on PROSPERO, while scoping reviews more often register their protocol on a general repository such as OSF because PROSPERO does not accept them. Either way, fixing the method in advance is what keeps the work reproducible.

How the two designs work in sequence
The methods are not only alternatives; they often run in order. A scoping review on an immature field surfaces the clusters where studies are plentiful and comparable, and those clusters become candidate questions for a later systematic review. This is why a protocol-registered mapping study is often the most efficient first move on an unfamiliar topic. Conversely, attempting a systematic review too early, before a scoping review has shown whether comparable studies exist, is the most common way a review collapses partway through, when the team discovers the included studies cannot be combined. Two errors dominate. The first is running a scoping review when stakeholders want an answer: a guideline panel that needs to know whether to recommend a treatment will not accept a map of what has been studied, and the work has to be redone as a systematic review with appraisal and synthesis. The second is the reverse, forcing a systematic review onto a scattered literature, which produces a thin review with too few comparable studies to support a credible conclusion. Matching the design to the underlying question before any searching begins is the single decision that prevents both.
