An umbrella review is a review of reviews: instead of collecting primary studies, it systematically gathers and appraises existing systematic reviews and meta-analyses on a topic, then summarises their findings side by side. It sits at the top of the evidence pyramid and is used when a question already has several reviews, so the goal becomes reconciling what those reviews collectively show rather than re-running the evidence from scratch.

When to choose a review of reviews

An umbrella review earns its place when a field is mature enough that multiple systematic reviews already exist, often with overlapping or even conflicting conclusions. Rather than adding yet another primary review, you map the existing reviews, compare their methods and results, and give decision-makers a single high-level summary. It is the right tool when a broad question spans several interventions or outcomes, each already reviewed, and a reader needs the big picture. If no reviews exist yet, you need a standard systematic review instead, and if you only want to map the landscape of evidence types, a scoping review is more appropriate.

How an umbrella review is conducted

1. Frame the question and write a protocol

The question is usually broad and is fixed in a protocol before searching, with the unit of analysis being a published review rather than a trial. Many teams register the protocol on PROSPERO to timestamp the methods.

2. Search for reviews, not primary studies

The search strategy targets systematic reviews and meta-analyses, often by adding review-type filters to the databases searched. As always, the search combines subject headings and free-text terms with Boolean operators and is documented for reproducibility.

3. Screen and select the reviews

After de-duplication, two reviewers screen records in duplicate against eligibility criteria, counting every stage for the flow diagram. Only reviews that meet the inclusion rules proceed.

4. Appraise the methodological quality of each review

Instead of rating individual studies, an umbrella review appraises each included review with a tool built for that purpose, most commonly AMSTAR 2. Where the underlying reviews report it, the GRADE certainty of evidence is also carried through so readers can weigh the strength of each conclusion.

5. Extract data and handle overlap

Reviewers extract the key characteristics and results of each review using a piloted extraction form. A distinctive challenge is overlap: the same primary studies often appear in more than one included review, which can double-count evidence. This is assessed and reported transparently, for example by mapping which primary studies feed which reviews.

6. Synthesise and report

Findings are usually combined in a structured narrative synthesis because re-pooling effect estimates across reviews risks double-counting. The summary compares the direction, size, and certainty of each review’s conclusions, and the whole report is written against the PRISMA 2020 guideline.

A worked example: reconciling conflicting reviews

Imagine a clinical team weighing whether vitamin D supplementation prevents respiratory infections. A search turns up six existing systematic reviews, three concluding a modest benefit and three finding none. Rather than run a seventh primary review, an umbrella review charts why they disagree: which doses and populations each covered, how each rated its included trials, and which shared the same underlying studies. Often the conflict dissolves once you see that the positive reviews pooled daily dosing in deficient populations while the negative ones pooled bolus dosing in replete populations. The umbrella review’s value is precisely this reconciliation, turning a contradictory literature into a structured explanation a decision-maker can act on.

Why overlap is the defining methodological challenge

Because included reviews frequently share primary studies, the same trial can be counted several times if you simply tally what each review reports. Left unaddressed, overlap inflates the apparent evidence base and can make a single influential trial look like independent confirmation across several reviews. The standard response is to build a citation matrix mapping every primary study to each review that includes it, then quantify the redundancy with the corrected covered area, a published metric that expresses how much of the evidence is repeated. High overlap does not invalidate an umbrella review, but it must be measured and reported so readers can judge how much independent evidence actually exists. How an umbrella review differs from neighbouring designs

Three errors recur. The first is ignoring overlap and presenting the included reviews’ conclusions as if each were independent, which double-counts shared trials. The second is re-pooling effect estimates across reviews to produce a single headline number, which compounds the overlap problem and is rarely defensible; a structured comparison of the reviews’ conclusions is the safer output. The third is skipping the quality appraisal of the reviews themselves, which lets a weak or biased review carry the same weight as a rigorous one and quietly corrupts the summary.

Strengths and limits to keep in mind

An umbrella review gives a fast, high-level read on a well-studied question and highlights where reviews agree or clash.
